Laparoscopy is a method of performing surgery in a minimally invasive manner through several tiny abdominal incisions, instead of the single long one required by the transabdominal approach. A video camera is used to visualize the pelvic organs and instruments are utilized through the other tiny ports to complete the operation. Although more skill and dexterity is required for the surgeon to perform these laparoscopic procedures, the end result is reduced hospitalization, much less pain, and a quicker, more comfortable recovery. There is also less scarring and adhesion formation and significantly faster return to normal life and full activities. 

Many patients whose diagnosis suggests traditional treatment, including an abdominal hysterectomy or open approach to remove the uterus, ovarian cysts, tumors, or pelvic mass, are actually candidates for a laparoscopic procedure. In fact, most GYN problems can and should be addressed in the most minimally invasive manner that the vaginal or laparoscopic route permits. 

More than 90 percent of the major surgeries we perform are via the minimally invasive laparoscopic route. We feel strongly about providing our patients with these beneficial procedures, progressing the art of advanced operative laparoscopic surgery, and teaching and sharing these skills with other gynecologists striving to learn these techniques.

Nonsurgical Alternatives

Essure

Essure is a new, quick, painless, permanent female sterilization procedure that can be easily and conveniently performed in a physician’s office. The Essure procedure can be performed without general anesthesia and does not require incisions. Many women drive themselves to and from the procedure and most return to normal activities the following day – some of our patients have even gone back to work the same day. During the procedure, a thin telescope is inserted through the vagina and cervical opening into the uterus. A small flexible device called a micro-insert is placed into each fallopian tube. Once the micro-inserts are in place, body tissue slowly and painlessly grows into the micro-inserts, blocking the fallopian tubes and preventing sperm from reaching and fertilizing the egg – thereby preventing pregnancy.

Dr. Chaiken is one of a small number of physicians nationally who has been specifically certified by Essure’s manufacturer as an Essure Accredited Physician. Certification signifies his exceptional experience and expertise in performing this procedure. Dr. Chaiken was the first gynecologist in the Baltimore region and one of the first in the Baltimore/Washington area to perform Essure. He also teaches and acts as a preceptor to other OB/GYN physicians in the region seeking to learn the operation. Dr. Chaiken is one of only a very few gynecologists in the nation who is regularly performing the Essure procedure in the comfort of his office.

Essure

Endometrial Ablation

Endometrial ablation is a procedure used to treat heavy menstrual bleeding for women who no longer wish to become pregnant. When medical therapy for persistent, heavy or abnormal uterine bleeding fails, hysterectomy has historically been the common treatment option. While hysterectomy may be the only answer for some, many women can avoid surgery and alleviate their bleeding with an ablation procedure that gently removes the uterine endometrial lining using a form of energy. The procedure is over 90 percent effective in reducing bleeding to normal levels and about 75 percent effective in permanently eliminating menstrual bleeding altogether. 

A variety of energy forms are used, including heat, electrical, or microwave. A woman’s uterine shape, configuration, and other pathology determine which form is used. Unlike a hysterectomy, which requires a hospital stay, general anesthesia, and weeks of recuperation, endometrial ablation can be performed in a physician’s office under a local block. The procedure usually takes ten minutes or less and most women resume normal activities within one or two days.

Different women will be better candidates for different treatments. Dr. Chaiken will work with you to determine the treatment that is right for you. He has expertise in the following systems and has helped to teach them to other gynecologists, regionally and nationally. He is also one of a very few gynecologists in the nation who performs these procedures in the comfort of his office and often is asked to speak on that subject.
